How to Transfer Your Treatment to ALPHA

If you’re currently being treated for Low Testosterone, Weight Loss, or any services Peptide Therapy at other clinics, it’s easy to transfer treatment over to ALPHA Men’s Health.

IMPORTANT! For TRT Clients: PLEASE DO NOT GET A TESTOSTERONE INJECTION WITHIN 5 DAYS OF YOUR APPOINTMENT WITH APLHA.

What you’ll need to transfer treatment:

  1. Copy of your most recent lab work (within 3 months)
    If you do not have lab work taken within 3 months, we will need to draw and send external labs

  2. Details of any Prescriptions that you currently have related to treatment that require transfer. Bring cell phone pictures if possible.

  3. Details of your current Weight Loss or Testosterone dosage information, so that our providers may take this into consideration when defining your ongoing treatment plan. For any at-home medications, please include photographs of these clearly showing the label and that the medication bears your name as the patient.

Make an appointment (select type ‘FREE T-Levels Check and Consultation’, or ‘FREE Weight Loss Consultation’ for Weight Loss), and bring the information listed above to your initial consultation appointment. You will be prompted during the appointment booking process to indicate that you’re a transferring client.
